I had an XP-100 fire a round as I was opening the bolt. Thankfully I had just started to lift the bolt handle so the lugs were still probably 98% engaged and the round went straight down range so the only damage was to my nerves. The gun was a custom build with a very light competition trigger and I think there was some friction somewhere. This happened a couple of years ago and the gun hasn’t been loaded since but I haven’t worked on it at all so it’s still locked away.
This was the worst issue I’ve had personally. As a professional gunsmith I’ve seen almost every malfunction you could imagine.
Had a nipple blow out on an under hammer M/L rifle. Blew the hammer back to half-cock and some powder burns on my left forearm. Wrong nipple stripped threads. It seemed tight. It wasn't.
